Various Health Reports:
Please be sure to bring or have your primary care physician (PCP) fax a copy of any reports pertinent to your care and reason for consultation. These documents may include Ultrasound / CT scan / Biopsy and Pathology / Mammogram / Endoscopy reports. Please remember to bring your insurance card and a copy/list of your most current medications.
Referrals Notifications:
Please check with your PCP if a referral is needed to see the doctor. If needed, verify with your physician that this referral was sent or received.
When to Arrive:
Please arrive 10–15 minutes prior to your scheduled appointment time.

Co-payments are collected at the time of service. For your payment convenience, the office accepts cash, check, MasterCard, Visa and American Express.
